SUMMARY

This is nursing work at the full performance level. Work involves the care and treatment of individuals in a group home and
sheltered workshop setting. Work is performed under the supervision of the Registered Nurse Supervisor. Assignments are
carried out in accordance with general work instructions and established nursing practices, procedures, and precedents.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING. OTHER DUTIES MAY BE ASSIGNED.
• Makes monthly and PRN visits to individuals with diabetes, seizure, high blood pressure, breathing treatments, feeding
tube, and colostomy care.
• Performs specialized treatments.
• Explains procedures and treatments to individuals to gain cooperation, understanding and allay apprehension.
• Administers prescribed medications and treatments in accordance with approved nursing techniques.
• Maintain awareness of comfort and safety needs of individuals.
• Observes individuals, records significant conditions and reactions and notify Registered Nurse Supervisor or Physician of
patient's condition and reaction to drugs, treatments, and significant incidents.
• Review all medical visits information for proper follow up.
• Review, track and document all individual's physicals and immunizations.
• Review all blister packs and MARs monthly.
• Review all medical records once every two (2) months.
• Accompany individuals on special doctor appointments.
• Act as a liaison between agency and pharmacy.
• Administer Hepatitis B and flu shots to staff.
• Ability to prepare and maintain nursing and individual care records and reports.
• Inventories and orders medicine and supplies; maintains appropriate inventory records.
• Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of the
individuals when needed.
• Responds to l~e saving situations based upon nursing standards, policies, procedures, and protocol.
• Initiates and implements training to staff in area of CPR / AED / FA, diabetes, seizures, applying topicals, lifting,
administering enemas, colostomy care, and other medical problems as they arise.
• Manage medication administration, induding Medication Administration Training and Compliance, practicum observer
training and records, Carasolva, I E-MAR maintenance and training and medication error records and remediation.
• Review and have physician sign off on physical order forms at six (6) months.
• Participates in department or unit quality improvement activities.
• Must be flexible and willing to be "ON CALL."
• Ability to establish and maintain effective working relationships with physicians, associates, individuals, the public, and
families.
• Performs related work as required.
• Comply with all applicable federal, state, local laws and regulations, policies and procedures and reports any concerns
about compliance practices.
• Obtain knowledge of, and exercise objective judgment and decisive thinking, in complying with, and in the application of, all
organization and program specific policies, practices and procedures including ethics, business practices, and compliance
program elements.

Qualifications

EDUCATION AND/OR EXPERIENCE
Graduation from an accredited school of nursing. Mental Health experience preferred.

CERTIFICATES, LICENSES, REGISTRATIONS
Possession of or eligibility for a license as a Registered Nurse issued by the Pennsylvania State Board of Nurse Examiners and/ Valid Pennsylvania PN Licensure.
Must have CPI training.
